What is Medication Therapy Management (MTM)?

Medication Therapy Management is a specialized clinical service provided by licensed pharmacists to help patients get the best possible health outcomes from their medications. Rather than simply dispensing pills, our pharmacist conducts a thorough audit of all your prescriptions, over-the-counter remedies, vitamins, and herbal supplements to detect drug interactions, duplicate therapies, unnecessary costs, and preventable side effects.

Who should use MTM services?

MTM is highly beneficial for patients taking multiple daily prescriptions (polypharmacy), those with chronic conditions such as diabetes, hypertension, asthma, COPD, or congestive heart failure, patients who recently transitioned home from a hospital stay, or anyone experiencing troublesome side effects or high medication expenses.

Does Medicare Part D or private insurance cover MTM?

Yes. Under federal CMS guidelines, all Medicare Part D prescription drug plans are required to provide an MTM program at $0 copay for eligible qualifying enrollees. Many commercial health insurance plans and Medicaid (HUSKY Health CT) also cover pharmacist clinical consultations.

Can family members and caregivers participate in the consultation?

Absolutely. With patient consent or power-of-attorney authorization, caregivers, adult children, and home healthcare aides are warmly encouraged to join either in person in our private consultation room at 46 Broad St or via phone. We provide a written Personal Medication Record (PMR) and Medication Action Plan (MAP) for your home care records.
